Rebates are available only for the purchase of new high efficiency* water heaters installed on or after January 1, 2025. The water heater must be installed where electricity is supplied by Itasca-Mantrap. Rebate submittal must follow the guidelines as listed below. Itasca-Mantrap is not responsible for inaccurate information supplied by retailers/contractors. Rebate program is subject to change or cancellation without notice. Call Itasca-Mantrap to verify rebate program status. *Installed water heater must meet the energy factor of .90 or higher to qualify for the rebate. Please include verification of energy factor. **Combined capacity includes a combination of more than one water heater or the addition of a mixing valve to a larger capacity water heater. Verify application requirements with Itasca-Mantrap before purchase and installation of water heater.

• The water heater must be installed within Itasca-Mantrap’s service territory. • Incomplete forms will not be processed.
• Water heaters must be high-efficiency with an energy factor of .90 or higher. Please include a copy of proof of efficiency.
• Water heaters must be controlled on a storage water heater program, with a total combined capacity of 100 gallons or more.
• Include a copy of the original dated sales receipt.
• Complete all member information including your account number and sign this form.
